Forms: Electronic and Printed

For your convenience the following forms have been provided to help you distribute and customize forms for submitting your nominations. There are a set of MS Word documents for use by local societies to customize and distribute for local use before submitting their nominees on the national level.

There are two types of forms:

PDF (Portable Document Format)
viewable using an Adobe Acrobat® Reader software. Adobe offers a free run-time license to users, the software can be downloaded at the Adobe Web site.
Microsoft Word documents
These documents will work with MSWord 97-plus on both the Windows and Macintosh platforms. Information which would need to be customized (dates and contact) are highlighted bold and red within the document.

Also included is a Microsoft Word Form which you may duplicate onto diskettes and give to teachers for completion on their computers. This form is a protected document and is set up for you to customize and then distribute on the diskettes.
